The contact form and email are not a secure way to share health information. Please do not include symptoms, diagnoses, medications, substance use, or details about your treatment. To ask about services, give your name, how to reach you, and a good time to call. Sensitive information should be shared only on a consultation call, in a session, or on my secure client platform after we start work together.
Contact form. Name (required), email (required), phone (optional), state of residence, your consent to be contacted by email (required), your consent to be contacted by phone or text (optional), and a short message. The form is processed by Formspree.
Booking requests. This website does not offer online booking. Consultations are requested through the contact form, by email, or by phone.
Automatically collected. This website does not use analytics or tracking tools. The site's host, GitHub Pages, may record visitors' IP addresses for security purposes. Fonts are loaded from Google Fonts, which receives your IP address and browser information when a page loads.
Cookies. This website does not set cookies. When you submit the contact form, Formspree may use cookies or a spam-prevention check to process the submission.
I use it to answer your inquiry, schedule a consultation, and run the website. I do not sell your information. I do not use advertising or remarketing tools, and I do not use your information to advertise to you.
GitHub (website hosting), Google Fonts (font delivery), Formspree (contact form processing), and my email provider. They may use information only to provide those services. I may also share information when the law requires it.
Once you become a client, information about your care is kept in my records on a secure platform and handled as described in my Notice of Privacy Practices.
Inquiries that do not become a client relationship are kept for [PLACEHOLDER: retention period] and then deleted. Client information is kept under my records retention policy.
You may ask to see, correct, or delete information you submitted through this website, subject to legal limits. Contact me at the email above.
