These choices are open to residents of New Mexico, Virginia, Oregon, California, and Colorado, and to anyone else who has used this site. You do not need an account to make a request.
I do not sell personal information, share it for targeted advertising, or use it for profiling. There is no sale or sharing to opt out of, but you may still send an opt-out instruction and I will record it.
Email: privacy@annemetz.com.
Mail: Anne Metz LLC, Attn: Privacy Contact, PO Box 595, Arroyo Seco, NM 87514.
Phone: 434-531-2396. Leave a message with your name and a way to reach you.
Tell me which choice you are making and the email address or phone number you used when you contacted me, so I can find your information. I will verify that the request comes from you using what I already have; I will not ask for more than I need. Please do not send a copy of an identity document, and do not include health information, in email. If verification needs more than your contact details, I will arrange a secure way to do it.
Someone acting for you (an authorized agent) may submit a request with evidence of their authority. I may confirm the request with you directly.
I will acknowledge your request within 10 business days and respond within 45 calendar days. If a request is complicated I may take up to 45 more days, and I will tell you why within the first 45. Requests are free up to twice in a twelve-month period. If I cannot do what you ask, I will explain why and tell you how to appeal.
If I deny a request, you may appeal by replying through any of the channels above with "Appeal" in the subject line, within 60 days of my decision. I will answer the appeal in writing within 45 days. If I deny the appeal, I will give you contact information for your state Attorney General and, for California residents, the California Privacy Protection Agency. You may contact those offices at any time without appealing to me first.
If you are or were a client, your counseling records are governed by health privacy law and my Notice of Privacy Practices, not by this page. You have separate rights to inspect and obtain copies of your records, request amendments, and receive an accounting of certain disclosures. Those requests are not subject to the response times above; I will meet the shorter deadlines that apply to health records. Contact me through the same channels and say that the request concerns your clinical record. I cannot delete a clinical record that I am required to keep.
